About the Role

Form Health is a virtual obesity medicine clinic seeking Physicians specialized in Endocrinology and Obesity Medicine to join our multidisciplinary chronic disease management team. This role focuses on the safe and effective management of patients with complex co-morbidities and integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy. The position includes participation in an after-hours call schedule with reduced clinical duties during on-call weeks.

Responsibilities

Provide direct patient care in a telemedicine setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management.
Manage a panel of patients and collaborate with APP colleagues and Registered Dietitians.
Review charts and conduct initial and return visits for APP patients to ensure quality and continuity of care.
Provide advanced treatment planning and medical management support for complex endocrinology cases.
Prescribe non-controlled medications for patients under direct care and in coordination with APPs.
Conduct weekly collaborative meetings and asynchronous communication with assigned Endocrine-Specialized APPs.
Ensure clinical documentation, protocols, and treatment pathways are followed consistently.
Assist in the professional development of colleagues by fostering clinical growth and offering feedback.
Participate in team-based care initiatives and contribute to a culture of collaboration.
Attend departmental meetings and contribute to quality improvement efforts.

Requirements

Board certification in Endocrinology by the American Board of Internal Medicine (ABIM).
Active and unrestricted Medical State License in any US State, with NEW YORK (NY), VIRGINIA (VA), or MASSACHUSETTS (MA) preferred.
Multi-state practice capability is required.
Must hold a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meet prerequisites to secure one.
Demonstrated competency in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.
Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens.
Strong interest in mentoring, teaching, and collaborating with APPs and Registered Dietitians.
Demonstrated experience working with APPs, medical residents, or trainees.
Excellent communication and interpersonal skills.
Adherence to evidence-based treatments for endocrine disorders and obesity.
Commitment to delivering patient-centered, culturally competent care.
