About the Role
Form Health is seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity. Physicians will collaborate with advanced practice professionals and registered dietitians to deliver exceptional team-based care in a remote setting with flexible schedules. This role is for a FTE (40 hours per week) opportunity, requiring licensure in New York, Virginia, or Massachusetts. It includes participation in a rotating after-hours call schedule with reduced clinical duties during on-call weeks.
Responsibilities
- Provide direct patient care focusing on evidence-based obesity treatment and chronic weight management.
- Collaborate with Registered Dietitians.
- Prescribe non-controlled obesity medications for patients under direct care.
- Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
- Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
- Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
- Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.
Requirements
- MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology.
- Licensed to practice and prescribe legally in NEW YORK (NY), VIRGINIA (VA), or MASSACHUSETTS (MA).
- Must have multi-state practice capability, either holding a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meeting prerequisites to secure one.
- Credentialed with insurance payers (preference for multiple state licenses).
- Excellent interpersonal skills for patient and team interactions.
- Committed to the highest quality of medical care and an evidence-based approach to obesity management.
- Prior experience conducting telemedicine visits with a great "web-side" manner.
- Prior experience working collaboratively with advanced practice professionals and registered dietitians.
- Bilingual candidates with medical fluency in Spanish are highly preferred.
