About the Role
The Advanced Practice Professional (APP) coordinates and manages the comprehensive care of hospitalized patients, collaborating with medical/surgical specialty teams to develop and review care plans. The APP performs diagnostic and therapeutic procedures, manages peri-operative services, and coordinates ambulatory management and discharge planning. They also participate in the education of residents and medical/nursing students.
Responsibilities
- Manages comprehensive and individualized care for hospitalized patients and their families.
- Completes daily rounds, consults, and pre-procedure/evaluations.
- Completes integrated assessments.
- Orders additional tests or consults in collaboration with hospitalist faculty or following established guidelines.
- Performs and communicates results of diagnostic tests.
- Consults and communicates with clinic and hospital staff regarding abnormal findings.
- Performs succinct comprehensive clinical evaluations and documents findings.
- Facilitates patient discharge by coordinating services and tests for outpatient follow-up.
- Develops a comprehensive individualized plan of care, including teaching and discharge planning.
- Demonstrates expertise in caring for patients across the age continuum.
- Identifies and facilitates intervention for unstable or complex patient care situations.
- Recognizes emergency situations and initiates effective emergency care.
- Evaluates general medicine patients and coordinates follow-up, procedures, and tests in the ambulatory setting.
- Assists in the collection of outcome measures.
- Demonstrates critical thinking and diagnostic reasoning skills.
- Demonstrates the ability to utilize equipment safely.
- Bases decisions and actions on scientific principles, standards of care, and research findings.
- Utilizes teaching/learning theory for education to staff, students, patients, families, and the community.
- Promotes an environment that facilitates learning.
- Updates own knowledge base.
- Completes all identified clinical competencies.
- Serves as a resource for staff caring for hospitalized patients.
- Provides educational programs to professional and paraprofessional personnel.
- Participates in community programs and distributes literature.
- Documents the plan of care, interventions, and evaluations clearly and concisely.
- Collaborates with other leaders and staff to achieve patient outcome improvements.
- Provides leadership in the development and implementation of changes in cardiology practice.
- Works collaboratively with medical and nursing staff for problem-solving and professional support.
- Participates and initiates inter-disciplinary approaches to patient care, program development, and education.
- Promotes and practices innovation in the delivery of care.
- Participates and provides leadership for committees and task forces.
- Documents hours and submits billing reports, as appropriate.
- Reviews and interprets cardiology data and recommends/implements changes.
- Participates in clinical trials and research studies.
- Assists Hospitalist Faculty, Administrators, and Directors in collecting outcome measures.
- Performs other duties/projects as assigned.
Requirements
- Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing (for Nurse Practitioner).
- National Certification by one of the recognized APRN Certification Centers (for Nurse Practitioner).
- Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia (for Nurse Practitioner).
- American Heart Association BLS Certification required (for Nurse Practitioner).
- Bachelor’s Degree or Master’s Degree as a Medical/Surgical Physician Assistant required (for Physician Assistant).
- Graduate of an ARC-PA approved Physician Assistant training program required (for Physician Assistant).
- Certification by the National Commission for Certification of Physician Assistants required (for Physician Assistant).
- Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine (for Physician Assistant).
- American Heart Association BLS Certification required (for Physician Assistant).
