About the Role
Provide one-on-one guidance, support, education, referrals, coordination of care, and other assistance to patients as they move through the healthcare continuum.
Responsibilities
- Develops relationships with participating physicians and service providers.
- Liaises with medical providers.
- Guides patients to appointments.
- Coordinates the logistics for plan adherence-appointments.
- Supports patient family members.
- Maintains a safe environment, cognizant of environmental factors and infection control issues.
Requirements
- Ability to be scheduled for work based on operational needs of the hospital.
- Ability to deal tactfully and harmoniously with guests.
- Ability to work in compassionate, supportive, and collaborative ways with patients and families in an acute medical setting.
Skills
- Effective communication skills with all stakeholders (e.g., patients, employer staff, payer contacts).
- Good interpersonal and communication skills with the ability to handle the pressures related to serious life crises and chronic disease situations.
