About the Role
Girling Home Care of NY seeks an experienced Physical Therapist to conduct home visits in NYC, providing therapy services based on a physician's plan of care. This includes assessing patient status, developing care plans, and instructing patients and families.
Responsibilities
- Evaluates home environment.
- Assesses muscle strength, mobility, gait, and range of motion for rehabilitation potential.
- Directs physical therapy treatment.
- Instructs patients and families/caregivers on the use and care of therapeutic appliances.
- Determines priority needs for physical therapy.
- Reports patient reactions to treatment or changes in condition to the physician.
- Initiates physical therapy programs and instructs personnel and/or family/caregiver members.
- Trains patients in the use of prosthetic devices.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Prepares and submits clinical and progress summaries based on goal attainment.
- Participates in patient discharge planning.
- Provides physical therapy consultation to home families/caregivers when indicated.
- Provides in-service education programs for nursing organization personnel.
- Participates in peer consultation process.
- Supervises Physical Therapy Assistants according to policy and state regulations.
- Instructs, supervises, and evaluates home health aide care every two weeks when therapy is the only skilled service.
Requirements
- Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
- Licensed to practice as a physical therapist within the State, if applicable.
- Two years of appropriate experience as a physical therapist.
- Demonstrates good verbal and written communication and organization skills.
- Possesses and maintains current CPR certification.
- Must be a licensed driver with an automobile that is insured and in good working order.
