About the Role

The Case Manager provides supportive and direct intensive case management services to homeless families residing in the Shelter. This role involves assessing client needs, developing collaborative plans, and preparing clients for self-sufficiency and independent living.

Responsibilities

  • Meet or exceed quarterly performance targets for Public Assistance and Employment, Facility Length of Stay, and Exit and Housing Placements.
  • Abide by the requirements set forth in The Salvation Army Nondisclosure Agreement for Social Services Personnel.
  • Oversee a caseload of 25 families.
  • Provide supportive assistance to caseloads in identifying and resolving issues that interfere with finding permanent housing placement.
  • Actively engage clients in employment and public assistance outcomes.
  • Maintain accurate and detailed case progress notes for every client meeting and engagement.
  • Enter case notes in the approved computer database of client records.
  • Interface with various social service agencies for advocacy and/or education on new referral sources.
  • Arrange meetings for assessment and intake of newly assigned residents within 24 hours of admission.
  • Perform concrete casework requirements and referrals, including assessment and entitlement, in a timely manner.
  • Identify clients in need of external services, contact these agencies, and help establish relationships with other service providers.
  • Complete all required paperwork, including bi-weekly service plans, case management notes, referral sheets, and statistical requirements.
  • Input a minimum of 15 contacts per week in the database system.
  • Engage and reinforce clients in discussions about Residence rules, regulations, and Department of Homeless Service Code of Conduct and Client Responsibility.
  • Make recommendations for next step referrals and involuntary transfers for noncompliant clients or those not making significant progress towards exiting the homeless system.
  • Provide follow-up and resolution to all client incident reports and complaint reports.
  • Actively participate in all required client hearings and discharges.
  • Support clients' goal of finding long-term permanent housing.
  • Participate in weekly housing, intake, and case conferencing staff meetings.
  • Actively engage in weekly clinical supervision with Case Manager Supervisor.
  • Work one late day per week (12pm – 8pm) and one rotating weekend shift per month.
  • Assume the same position at another family shelter operated by The Salvation Army at the discretion of leadership.
  • Undertake other assignments and tasks as added at the discretion of leadership.

Requirements

  • Experience working with clients pursuing independence and self-sufficiency outcomes is preferred.

Skills

  • Bilingual - Spanish

Education Level

  • Bachelor’s degree in Social Work, Psychology, or a related field.
