About the Role
The Licensed Vocational Nurse (LVN) will provide medical expertise within Transitional Age Youth (TAY) and Adult Intensive Case Management Programs. This role involves psychiatric evaluation, health assessment, medication administration and monitoring, care coordination, and crisis intervention to promote client recovery, stability, and independence.
Responsibilities
- Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring.
- Works with an interdisciplinary team that includes clinical case managers, case managers, and other allied health professionals in the community.
- Provides therapeutic support and psychoeducation to clients, including harm reduction, relapse prevention, and motivational interviewing.
- Makes appropriate referrals and linkages to other services.
- Coordinates care with outside providers for comprehensive care.
- Maintains clinical records in accordance with Medi-Cal standards and Felton policies.
- Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care.
- Performs additional duties as assigned by supervisor or assignee.
Requirements
- Completion of training in an accredited Licensed Vocational Nurse Program.
- Active Licensed Vocational Nurse license from the California Board of Licensed Vocational Nurse.
- Experience working with SMI (severely mentally ill) populations and community mental health settings preferred.
- Demonstrated commitment to the welfare of individuals and families with mental and behavioral health issues.
- Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and psychosis.
- Complex problem solving, critical thinking, and social perceptiveness skills.
- Service orientation skills; actively looking for ways to help people.
- Demonstrated interest in ongoing learning and training.
- Ability to be a team player with a welcoming attitude.
- Willingness to travel for outreach to clients in the community.
