About the Role

The Licensed Practical Nurse administers nursing care to designated residents under the direction of a registered nurse. This role coordinates and supports the activities of certified nurse assistants and actively participates in direct resident care, following the specific approaches stated in the plan of care. Responsibilities

• Assess resident care needs and assist in the development of Individualized Service Plans (ISP) to encourage resident independence and self-esteem.
• Update and communicate ISP plans of care that identify resident health problems/needs and the goals to be accomplished.
• Administer and record treatments/medications.
• Document resident care, condition, and response to care.
• Clarify and implement physician orders.
• Determine the need to transfer a resident to the hospital and take necessary steps for a smooth transition.
• Coordinate re-admissions from the hospital.
• Communicate resident condition to physician, Director, and responsible party promptly and take appropriate action.
• Count medications, administer treatments/medications according to physician’s orders, and accurately record them.
• Provide direct resident care and clear instruction to aides.
• Participate in scheduling, be flexible, and willing to cover shifts as necessary.
• Provide direct resident care and clear instruction to care partners.
• Listen to resident, family, or physician concerns and implement appropriate action.

Requirements

• Two years' experience in SNF, acute care, or long-term setting.
• LPN License.
• First Aid Certification for Court/Terrace.
• CPR/BLS for GHRC for Nurses (RN’s & LPN’s).
