About the Role

Provides day-to-day oversight and supervision for the Family Care Program, ISS Program, and Families In Need Program, ensuring compliance with fiscal, regulatory, and agency policies. Acts as a representative of CDS Monarch for these programs.

Responsibilities

  • Ensures daily program compliance with OPWDD regulations and CDS Monarch Policy and Procedures.
  • Ensures timely submission of all re-certifications.
  • Assures completion of all paperwork and adherence to policies.
  • Ensures compliance with required staff training for all applicable programs.
  • Reviews monthly program documentation.
  • Maintains organized program records.
  • Completes requests and submits for payments.
  • Ensures compliance with all billing requirements.
  • Provides administrative on-call support for Family Care providers and individuals.
  • Attends annual and bi-annual case reviews, creates/revises protective oversight plans and residential habilitation plans.
  • Follows agency policy and procedure for record storage and retention.
  • Provides quality assurance inspections of Family Care records and homes.
  • Completes Family Care Provider payroll.
  • Attends monthly home visits.
  • Reviews and obtains approval for Families in Need requests.
  • Completes and submits bi-annual ISS reports.
  • Performs other duties as requested by the supervisor.

Requirements

  • Must be at least 18 years of age.
  • Adhere to all CDS Life Transitions, Inc. policies and procedures.
  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.
  • Attend mandatory education and training modules as scheduled; obtain and maintain required certifications.
  • Maintain all required certifications/training by State regulations and CDS policy.
  • Act as a professional representative of CDS Life Transitions, Inc. in regard to appearance, behavior, temperament, communication, language, and dress.

Skills

  • Knowledge of Medicaid Services Coordination and/or Broker.
  • Experience working with families and customer service.
  • Ability to work independently and motivate others.
  • Excellent communication skills.

Experience Level

  • Two years’ experience in Service Coordination with individuals with Developmental Disabilities.

Education Level

  • Bachelor’s Degree in a human service related field preferred.
