About the Role
Silver Tree Residential is seeking a compassionate and motivated Service Coordinator for Melrose Villa Hermosa, an 80-unit apartment community for seniors. The role involves upholding community standards and improving residents' quality of life by connecting them with services and fostering self-sufficiency.
Responsibilities
- Develop relationships and partnerships with agencies and nonprofits to provide resident services.
- Link and develop educational programming, wellness presentations, and on-site events for residents.
- Serve as a liaison with medical, professional services providers, and community agencies.
- Report abuse, neglect, or exploitation of a resident in compliance with state statutes and ethical standards.
- Maintain resident files in compliance with HUD and Silver Tree Residential requirements.
- Prepare and submit HUD regulatory reports as directed.
- Develop and maintain a resource directory for the apartment community.
- Assist residents, families, and designated representatives with transitions into and out of the facility when consumer-directed.
- Accompanies Community Director on apartment inspections and collaborates on plans for residents to age in place.
- Follow up on referrals from property management team inspections.
- Create a monthly calendar and site newsletter.
- Attend home visits of prospective tenants to discuss the Service Coordination program and assist with the move-in process.
Requirements
- Possess a minimum of three years of experience providing social services.
- Possess a genuine interest in continuing education that directly serves the elderly.
- Be informed about legal liability issues relevant to providing social services to at-risk populations.
- Possess a sincere commitment and passion to serve an elderly population characterized by physical, functional, social, and cognitive challenges.
- Possess strong written and verbal communication skills.
- Ability to problem-solve, educate, and advocate for vulnerable adults.
- Excellent organizational skills.
- Knowledge of MS Office.
- Experience with Excel spreadsheets is required.
