About the Role

Hot Bread Kitchen is seeking a temporary full-time Case Manager to provide direct support services to Front of House and Culinary Fundamentals Program participants. This role is critical in assessing member strengths and needs, connecting them to resources, and enabling meaningful participation in the programs. The Case Manager will collaborate with the training team and report to the Manager of Member Engagement.

Responsibilities

  • Facilitate member alumni services and/or events.
  • Follow up with participants post-program to assess outcomes and provide continued support.
  • Monitor the impact of member engagement activities and address program needs.
  • Participate in case conferences, team meetings, supervision, and professional development.
  • Identify and develop new referral partnerships to address member needs.
  • Teach professional readiness to members enrolled in career programs, facilitating workshops on goal-setting, resource navigation, and workplace readiness.
  • Support Front of House and Culinary instructors with classroom management and kitchen observations.
  • Advocate for members’ needs.
  • Use motivational interviewing techniques to guide participants toward goal achievement.
  • Maintain accurate, confidential, and up-to-date member records in Salesforce, including individualized service plans and progress notes.
  • Track participation, outcomes, referrals, and follow-up activities in the organization’s database.
  • Support community outreach efforts.
  • Identify and respond to urgent needs, crises, or safety concerns according to organizational protocols.

Requirements

  • Strong passion for Hot Bread Kitchen’s mission.
  • Bachelor degree in Social Work, Psychology, Human Services or related field required.
  • Direct service experience or 2-5 years experience in case management.
  • Experience working with and supporting diverse populations.
  • Some facilitation experience, preferably for teaching soft skills or professional readiness.
  • Computer proficiency in Salesforce, Microsoft Office Suite, and Google Workspace Suite.
  • Ability to think and problem solve creatively.
  • Ability to collaborate frequently.
  • Possess a sense of humor.
  • Familiarity with navigating NYC’s support systems (affordable housing, childcare, mental health, public benefits, etc.).
  • Must be legally authorized to work in the U.S. at the time of hire.
