About the Role
Assist individuals in developing independent living skills to increase their self-sufficiency, self-esteem, and self-worth. Provide hands-on training, assist with daily living activities, and offer counseling, behavioral intervention, and crisis management for adults living with severe mental illness.
Responsibilities
- Participate in the planning and evaluation of individual service plans and monitor plan objectives.
- Provide hands-on training and assist residents in daily living activities, documenting interactions.
- Accompany residents on recreational outings and travel training exercises.
- Provide counseling, behavioral intervention, and crisis management.
- Assist in the day-to-day management of the community residence.
- Administer the residence in the absence of the Assistant Administrator.
- Notify the Assistant Administrator/Residential Administrator of any medical/psychiatric emergency.
- Provide transportation for shopping, medical appointments, etc.
- Write daily progress notes on residents' activities.
- Monitor medication management.
- Maintain the cleanliness of the residence.
- Maintain the cleanliness of the staff office.
- Advocate for residents' benefits.
- Teach residents the skills needed to gain independence.
- Coordinate staff contacts with local service providers to assist residents in accessing services and plan for discharge.
- Work with support services on behalf of the residents' best interest.
- Assist and model techniques for maintaining a clean and healthy living environment.
- Perform other related activities.
Requirements
- High School Diploma or equivalent with related experience required.
- Bachelor's Degree in Human Services preferred.
- Related experience working with people living with mental illness or other disabilities that require assistance in day-to-day living highly preferred.
- Ability to sit, stand, stoop, squat, walk, and move intermittently during work hours.
- Ability to push or pull 10-15 pounds daily and up to 50 pounds occasionally.
- A valid NYS Driver's License in good standing.
