About the Role

Assist individuals in developing independent living skills to increase their self-sufficiency, self-esteem, and self-worth. Provide hands-on training, assist with daily living activities, and offer counseling, behavioral intervention, and crisis management for adults living with severe mental illness.

Responsibilities

  • Participate in the planning and evaluation of individual service plans and monitor plan objectives.
  • Provide hands-on training and assist residents with daily living activities, documenting interactions.
  • Accompany residents on recreational outings and travel training exercises.
  • Provide counseling, behavioral intervention, and crisis management.
  • Assist in the day-to-day management of the community residence.
  • Administer the residence in the absence of the Assistant Administrator.
  • Notify the Assistant Administrator/Residential Administrator of any medical/psychiatric emergency.
  • Provide transportation for shopping, medical appointments, and other needs.
  • Write daily progress notes on residents' activities.
  • Monitor medication management.
  • Maintain the cleanliness of the residence.
  • Maintain the cleanliness and organization of the staff office.
  • Advocate for residents' benefits.
  • Teach residents the skills needed to gain independence.
  • Coordinate with local service providers to assist residents in accessing services and plan for discharge.
  • Work with support services on behalf of the residents' best interest.
  • Assist and model techniques for maintaining a clean and healthy living environment.
  • Perform other related activities as needed.

Requirements

  • High School Diploma or equivalent with related experience.
  • A valid NYS Driver's License in good standing.
  • Ability to sit, stand, stoop, squat, walk, and move intermittently during work hours.
  • Ability to push or pull 10-15 pounds daily and up to 50 pounds occasionally.
