About the Role
Serve as a resident advocate, supporting each resident’s rights in the facility and assisting staff, residents, and their families with interpretation of those rights. Facilitate the adjustment of residents to the Facility and to the physical and psychological limitations which necessitated or precipitated their placement in the Facility.
Responsibilities
- Facilitate resident adjustment to the facility and their limitations.
- Assist residents and families in understanding and accepting limitations.
- Encourage residents to engage their strengths.
- Enhance staff understanding of residents’ psychosocial needs and motivations.
- Serve as liaison between residents, families, and facility staff.
- Visit and interview residents following admission to assist with adjustment.
- Compile written records of pertinent information.
- Prepare the resident’s social/psychosocial history, psychosocial assessment, and Social Service Care Plan within 14 days of admission.
- Assess discharge potential and intentions of each resident.
- Assist facility staff in understanding the significance of social, psychological, and economic factors on resident life and behavior.
- Assist residents and families in understanding the plan of care and encourage participation in revisions.
- Serve as resident advocate by assisting residents in interpreting and implementing their rights and responsibilities.
- Encourage residents to voice grievances and/or recommendations.
- Participate in seeking resolution to voiced concerns.
- Serve as liaison between the resident, the psychologist consultant, and the Interdisciplinary Team.
- Review and update psychosocial assessment, care plan, and discharge plan approximately every ninety (90) days, or more often as necessary.
Requirements
- Minimum of a Bachelor’s degree in Social Work or a related field.
- Minimum of two years of experience in the social services field, preferably in a nursing facility, hospital or related agency.
- Pleasant personality and ability to work with residents, families, and staff from various departments.
- Proficient in Microsoft Office.
- Excellent communication and organization skills.
- Ability to work well under pressure and be flexible.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents.
- Work well under pressure & willing to work harmoniously with all staff.
- Covid-19 vaccinations are encouraged but NOT MANDATED.
