About the Role

This role focuses on providing pharmacy related services in a hospital or retail setting. It involves reviewing and approving medication orders, monitoring drug therapies, and providing drug information to improve patient health. The position also serves as a resource to other health professionals, educates patients and staff, and supervises support personnel.

Responsibilities

  • Directs and coordinates the clinical activities of the assigned pharmacy service area.
  • Prepares, compounds, and dispenses medications according to state and federal laws.
  • Reviews and interprets physician’s orders for appropriateness, indication, clarity, dose, drug allergy, potential drug interactions, and duplication.
  • Confers with physicians regarding medication orders and documents interventions.
  • Ensures accurate preparation and labeling of medications.
  • Supervises support staff and ensures pharmacy operations comply with state and federal regulations.
  • Provides accurate, appropriate, and timely drug information.
  • Conducts medication history interviews and reconciles patient medication therapies.
  • Provides drug information to patients and their families.
  • Anticipates controlled drug needs based on Controlled Drug Disposition Records.
  • Completes medication station inspections.
  • Participates in medication education and documentation in patient progress notes.
  • Understands and participates in Medication Use Evaluation.
  • Performs order entry and utilizes all Registered Pharmacist functions of the Pharmacy computer system accurately and efficiently.
  • Enters all necessary information, including drug allergy information and information for charging.
  • Participates in multidisciplinary team meetings.
  • Reviews discharge medication lists from skilled nursing facilities and inpatient hospitalizations.
  • Improves medication reconciliation process.
  • Provides medication optimization consults for patients.
  • Conducts home visits for identified patient populations to review medications and provide education.
  • Participates in Pharmacy Quality Programs related to people, clinical excellence, operational excellence, financial performance and growth, safety and quality, and research.
  • Collaborates with other pharmacists to provide operational support, including weekend, holiday, evening, night, and on-call coverage.
  • Provides continuing education for clinical staff.
  • Serves as preceptor for pharmacy students, interns, and residents.
  • May participate in the development and implementation of treatment guidelines, protocols, and clinical pathways.
  • May participate in the development of departmental drug-specific and population-specific initiatives.

Requirements

  • Bachelor’s degree in Pharmacy.
  • Pharmacist License.
  • One (1) year of related experience.
  • Doctor of Pharmacy (PharmD).
  • Board Certification.
  • Four (4) years of related experience.
  • Clinical pharmacy expertise with direct patient care experience required, either through residency training or prior job experience.
  • Experience in drug utilization review and analysis.
  • Frequent standing & walking, and lifting of 5-10 lbs.
  • May require lifting and carrying light loads, including boxes, equipment, and stooping or kneeling.
  • Requires manual dexterity using fine hand manipulation to operate prescription medications and computer keyboard.
  • Requires ability to see medications, computer screen, and reports.

Skills

  • Knowledge of Pharmacy Laws, regulations and professional standards.
  • Ability to keep appropriate records.
  • Accurately maintains the Pharmacy Department perpetual inventory for controlled drugs.
  • Superior verbal and written presentation skills.
  • Superior communication skills, including developing and giving presentations, both 1-1 and group.
  • Knowledge of integrated health networks.
  • Knowledge of health plan drug formularies.
  • Project and resource management skills.
  • Creative thinking and problem-solving skills.
  • Ability to understand importance of and respect for the confidentiality of all patient information.
  • Knowledge of clinical best practices and standards for optimizing medication use.
  • Proficiency in Microsoft Office applications, including Access, Excel, PowerPoint and Word.
  • Ability to work with all ages, backgrounds, ethnicities, and life experiences.
  • A robust understanding of chronic health condition management and population management.
  • Motivational Interviewing proficiency.
  • Ability to prioritize and resolve critical issues efficiently and effectively.
  • Detail oriented, with strong organizational skills and multi-tasking abilities.
  • Very strong working knowledge and proficiency with technology and business software (Microsoft Office).
  • Experience with Electronic Medical Records.
  • Willingness and ability to learn and utilize new technology and procedures.
  • Ability to work independently with minimal supervision and as part of a team.
