About the Role
Community Health and Wellness, LLC is seeking Certified Therapeutic Recreation Specialists to provide services for individuals with disabilities throughout Texas. This role involves one-on-one recreation therapy, goal creation, documentation, and team communication.
Responsibilities
- Provide one-on-one home and community Recreation Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses.
- Create tailored goals to meet the needs of individual clients.
- Document on each session.
- Communicate with other team members and service providers.
- Turn in timely documentation including weekly invoicing, progress notes, and related paperwork.
- Implement patient care plans through home and community goal-related activities.
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
Requirements
- Must be able to travel to current or prospective clients’ homes.
- Must be able and willing to transport client(s) during client visits.
- Must have reliable transportation, a valid driver’s license and agency-required auto liability insurance (100k/300k Bodily Injury).
- Must be able to communicate with health care professionals, clients, families, and the general public through verbal and written communication.
- Must have access to a private personal laptop or desktop computer for HIPAA compliance.
- Able to work independently, manage one’s own time and the time of client(s).
- All contractors must submit to background and reference checks.

Education Level
- Minimum of Bachelors Level Degree required in one of the following: Therapeutic Recreation, Recreation Administration with emphasis in Therapeutic Recreation, Leisure Studies with emphasis in Therapeutic Recreation, and/or related degree to Therapeutic Recreation.
