About the Role
Community Health and Wellness, LLC is seeking an Independent Contract Music Therapist to provide one-on-one music therapy services to children and youth with congenital disorders, intellectual disabilities, and physical disabilities.
Responsibilities
- Provide one-on-one Music Therapy services to children and youth with congenital disorders/intellectual disabilities/physical disabilities
- Create tailored goals to meet the needs of individual clients
- Document on each session
- Communicate with other team members and service providers
- Turn in timely documentation including weekly invoicing, progress notes, and related paperwork
- Travel to current or prospective clients’ homes
- Communicate with health care professionals, clients, families, and the general public through verbal and written communication
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
- Implement patient care plans through home and community goal-related activities
- Work independently, manage one’s own time and the time of client(s)
Requirements
- Must have access to a private personal laptop or desktop computer for HIPAA compliance
