About the Role
Community Health and Wellness, LLC is seeking professional Aquatic Therapists to provide services for individuals with disabilities throughout Texas. This role involves one-on-one aquatic therapy, goal setting, documentation, and collaboration with team members.
Responsibilities
- Provide one-on-one Aquatic Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses.
- Create tailored goals to meet the needs of individual clients.
- Document on each session.
- Communicate with other team members and service providers.
- Turn in timely documentation including weekly invoicing, progress notes, and related paperwork.
- Travel to current or prospective clients’ homes.
- Transport client(s) during client visits.
- Communicate with health care professionals, clients, families, and the general public through verbal and written communication.
- Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
- Implement patient care plans through home and community goal-related activities.
- Work independently, manage one’s own time and the time of client(s).
Requirements
- Must have reliable transportation.
- Must have a willingness to travel to consumer homes and facilities.
- Must have access to a private personal laptop or desktop computer for HIPAA compliance.
- Must submit to background and reference checks.
