About the Role

The African American Planning Commission, Inc. (AAPCI) is seeking a Mental Health Counselor to join their team at Edwin's Place. This role focuses on reducing homelessness and addressing complex issues such as domestic violence, HIV/AIDS, mental illness, substance abuse, and housing shortages. Responsibilities

Serve as part of the Intake/screening/Assessment team to determine client suitability for independent living and housing program placement.
Provide individual and group counseling on substance abuse, mental health, or co-occurring issues.
Co-lead specific resident population groups based on assigned area.
Utilize a Person-Centered approach to assist clients in setting goals, identifying interests, and addressing concerns.
Develop strategic interventions and services by understanding clients and their psychosocial history.
Educate adults and families, modifying treatments as necessary based on client progress.
Develop and implement treatment plans based on clinical experience and knowledge.
Employ psycho-educational techniques to help clients understand and adhere to medication regimens and advocate with their providers.
Participate in educational activities and department meetings.
Conduct assessments and psychological evaluations.
Refer adult clients and/or families to appropriate community agencies and resources for assistance, counseling, rehabilitation, or auxiliary care.
Prepare, write, and maintain all required progress notes, reports, and group notes.
Maintain all electronic records as required.
Coordinate phone and triage activities.
Regularly coordinate care with collateral contacts, including family, providers, ACS, APS, and Schools.
Maintain a strengths-based perspective for family clients to improve family functioning.
Conduct client unit visits to ensure compliance with lease and program rules.
Enlist property management support for wellness checks.
Conduct and maintain Monthly Progress Notes/Treatment Reports.
Lead/Co-Lead monthly tenant meetings.
Provide group activities and services, including educational and recreational activities.
Develop and maintain contacts with community agencies to gather resource information, relay client information, and improve services.
Assist clients in understanding and proactively addressing diagnoses (medical, psychiatric, substance abuse, etc.).
Perform other related duties as requested by the supervisor.

Requirements

Master’s degree in a related field (social work, mental health counseling, psychology, sociology etc.).
At least 2-3 years’ experience in a residential setting.
Familiarity with NYC Supportive Housing.
Familiarity with mental health, substance use, HIV/AIDS, or any other related field.
Strong verbal and interpersonal communication skills with a focus on providing excellent client services.
Demonstrated ability to interact effectively and collaboratively with a diverse community of residents, program staff, and external partners.
Ability to exercise good judgment and apply problem-solving skills.
Experience working collaboratively in a team-oriented and outcomes-focused environment.
Ability to monitor and manage a variety of assigned tasks.
Must be able to work full-time.
Must be able to work morning shifts.
Must be able to travel up to 25%.
