About the Role

This is a paid, educational internship designed to support the development of clinical and case management skills in a supervised learning environment. Interns will gain hands-on experience in assessment, care planning, and service coordination while working closely with experienced care managers.

Responsibilities

  • Develop a comprehensive learning agreement to establish educational goals and objectives.
  • Complete required onboarding, including Workday training, documentation training, and shadowing visits.
  • Co-manage a caseload of approximately 10-15 clients alongside a Care Manager mentor.
  • Assist in conducting comprehensive client assessments, including psychosocial, physical, mental health, environmental, and spiritual needs.
  • Support the development and implementation of client-centered care plans (Community Living Plans).
  • Assist with identifying, coordinating, and monitoring appropriate community-based services.
  • Learn and apply knowledge of Medicare, Medi-Cal, and other entitlement programs relevant to the population served.
  • Build and maintain professional relationships with clients, caregivers, and service providers while demonstrating respect, dignity, and professionalism.
  • Participate in client visits (minimum monthly or as appropriate), with guidance from supervisor.
  • Learn and follow agency documentation standards, including timely and accurate progress notes (within 48 hours).
  • Maintain organized and accurate records to support continuity of care.
  • Support client and caregiver education and empowerment strategies.
  • Participate in evaluating the effectiveness of interventions.
  • Assist with case transitions, including closure or transfer at the end of the internship.
  • Participate in interdisciplinary team (MDT) meetings, staff meetings, and trainings as assigned.
  • Maintain regular communication with supervisors and actively engage in supervision.
  • Complete all required academic documentation in collaboration with Field Instructor.
  • Adhere to the ethical standards of the National Association of Social Workers (NASW).

Requirements

  • Ability to perform essential job functions with or without reasonable accommodation.
  • Ability to conduct field-based work, including traveling to client locations.
  • Ability to use standard office equipment and complete electronic documentation.
  • Ability to communicate effectively in English for client care, documentation, and collaboration with care teams.
