About the Role
The HCV Navigator serves as the point of contact for all patients engaged in hepatitis C testing and treatment at REACH. The navigator will work with the REACH Hepatitis C nurse to case-manage and coordinate all elements of the REACH hepatitis C program, aiming to provide services across New York State via telemedicine. Core responsibilities include programmatic education, intake, testing, treatment, patient navigation, and management of the patient incentive program, all while addressing and empowering participants' efforts to reduce risk, screen, and treat Hepatitis C.
Responsibilities
- Work collaboratively with front office and clinical staff to appropriately identify potential participants.
- Build trust, reduce stigma, and support patients in getting tested, accessing treatment, and adhering to medication.
- Facilitate Hepatitis C panel initial and post treatment testing, navigating any issues that might keep a patient from getting the testing.
- Provide pre and post-test counseling and education.
- Work with the Hepatitis C Nurse to efficiently schedule provider telemedicine visits for hepatitis C positive patients.
- Work closely with Hepatitis C positive patients as they navigate treatment, assuring they receive and take the appropriate medications.
- Report any issues with lab testing, medication side effects, and missed doses of medications to the Hepatitis C Nurse.
- Work with the Hepatitis C Nurse to maintain appropriate aggregate patient demographics, treatment steps, incentives, and any other needed metrics.
- Work with the Hepatitis C Nurse to assure that all aspects of individual patient treatment are documented in the REACH electronic medical records system clearly and accurately.
- Other duties as assigned by the Hepatitis C Nurse.
- Provide excellent customer service at all times.
- Ensure program deliverables are met.
- Participate in on-going trainings as needed.
- Follow universal safety precautions at all times.
- Attend and participate in local community events as needed.
- Complete accurate and timely submission of data reports, including data entry & submission of performance tracking reports.
- Meet at least 90% of all monthly HCV/HIV testing related deliverables as outlined on the contract work-plan.
Requirements
- Individuals with lived or living experience of Hepatitis C, substance use, or navigating the healthcare system.
- Strong organizational and collaborative skills.
- Experience utilizing electronic medical records.
- Must have extensive (2-4 year) experience utilizing Cloud based information systems.
- Must have extensive (2-4 year) experience utilizing Excel.
- Demonstrate a commitment to harm reduction theory and practice.
- Demonstrate a commitment to advocacy, particularly in the case of HIV/AIDS, hepatitis C, homelessness and drug policy.
