About the Role
Under general supervision, this role is responsible for developing and implementing Individualized Service Plans for individuals and their advocates within a residential setting, ensuring compliance with agency regulations. It involves providing case management, identifying needs, linking individuals with appropriate service providers, and monitoring satisfaction.
Responsibilities
- Provides social services to maximize social functioning and enhance coping capacity.
- Develops and monitors individual treatment plans in accordance with regulatory and agency policy.
- Participates in and chairs interdisciplinary planning meetings, interpreting and following up on recommendations.
- Establishes communication channels with staff to interpret psychological needs of residents.
- Develops goals and activities generated by the interdisciplinary team, producing an active treatment plan schedule.
- Documents comprehensive functional assessments and reviews of the plan of care.
- Ensures proper data collection through observation and in-service training.
- Assists in activities of daily living and participates in off-site activities.
- Supervises Residence Program Specialists in the implementation of active treatment plans.
- Makes regular contacts with families, residents, and agency-wide.
- Trains and instructs Residence Program Specialists on individual goal plans, data collection, and daily routines.
- Maintains contact with social service agencies to access resources and advocate for services.
- Performs individual evaluations to determine the need for alternate placement.
- Handles family outreach and in-service training.
- Ensures timely application for resident benefits and transfer of funds.
- Provides systematic follow-up for referrals to community agencies upon discharge.
- Maintains security of confidential information, exercising discretion.
- Coordinates admissions, including referral material preparation, screening, and contact with families.
- Ensures required adaptive equipment is secured and maintained.
- Maintains up-to-date files on applicants.
- Processes new admissions, including family contact and day programming arrangements.
- Advocates for residents to ensure their rights are fulfilled.
- Writes monthly case notes on client progress and family contacts.
- Serves as a liaison to Day Programs, Service Coordination, and Clinics.
- Stays informed about changes in the field.
- May be mandated to work beyond scheduled shift based on operational needs.
- Performs other related duties as requested.

- Exposed to a normal office setting with little or no discomfort due to noise or temperature.
- Some walking, bending, and standing required for tasks like photocopying and accessing files.
- Work may be performed under pressure and can be mentally fatiguing.
