About the Role
Wellstar is seeking a Care Partner/Nurse Assistant to provide support care under the direction of a Registered Nurse. The primary focus is to promote a positive patient experience by assisting with daily living activities and comfort measures, ensuring excellence in patient care and safety.
Responsibilities
- Provide patient care under the direction of the RN.
- Assist with patient hygiene, nutrition, elimination, and comfort measures.
- Perform advanced skills as required by unit needs (e.g., phlebotomy, EKG, Foley catheter removal, IV removal).
- Answer telephone and call lights promptly and professionally.
- Assist RN and healthcare team with care delivery.
- Assist in direct patient admission.
- Monitor data entry for key patient safety information.
- Participate in charge reconciliation and perform audits.
- Transport unit supplies, patient care supplies, and medications.
- Use effective written and oral communication skills with patients, staff, physicians, family, and visitors.
- Use safety communication techniques (ARRC).
- Communicate completion of assignments to RN or Charge Nurse.
- Document patient care in the EMR.
- Communicate patient care concerns/problems/emergencies to the RN or Charge Nurse in a timely manner.
- Maintain and clean patient care equipment.
- Communicate with patients and update white boards.
- Provide a safe environment for patient care.
- Practice proper hand hygiene.
- Keep patient rooms and unit areas clean and free of clutter.
- Follow appropriate PPE/Infection control policies.
- Reinforce the importance of universal precautions.
- Utilize proper body mechanics and patient safe handling devices.
- Perform environment of care rounds.
- Protect patient rights and dignity.
- Maintain confidentiality adhering to HIPAA regulations.
- Participate in hourly patient rounding.
- Resolve patient concerns or escalate to the charge nurse.
- Proactively offer assistance and comfort measures to patients and families.
- Present self in a positive and professional manner.
- Support initiatives of the Health care team.
- Participate in unit performance improvement initiatives.
- Support professional nursing practice.
- Practice accountability.

- Must be able to take direction from a variety of care providers.
- Must be organized with excellent communication skills.
- Wellstar minimum age of hire is 16.
