About the Role

The Care Manager oversees and coordinates access to services for people with intellectual and developmental disabilities. This role involves developing, implementing, and monitoring a person-centered Life Plan to ensure individuals lead active, healthy, and fulfilling lives.

Responsibilities

• Deliver person-centered care management services in compliance with regulatory standards and agency policies.
• Complete comprehensive assessment and reassessment processes.
• Identify service gaps and make appropriate referrals.
• Advocate for members to achieve their goals.
• Develop, implement, and monitor member Life Plans by leading interdisciplinary team processes.
• Develop strategies to resolve conflicts within the person-centered planning process.
• Complete all required service documentation within stated timeframes, ensuring billing accuracy.
• Maintain member eligibility for care management through annual Level of Care (Re)Determination.
• Identify and access eligible benefits and entitlements, and ensure existing benefits are maintained.
• Ensure accurate and up-to-date information sharing consent in the electronic health record.
• Coordinate and provide access to high-quality healthcare services, including medical and behavioral health.
• Coordinate transitional care, including discharge planning and transfers.
• Utilize health information technology, including electronic health records and telehealth services.
• Attend department meetings, trainings, and supervisions.
• Complete all required trainings within specified timeframes.
• Travel to meet with providers, team members, and accompany members to appointments.
• Follow incident reporting guidelines to ensure member safety.
• Maintain confidentiality in accordance with HIPAA.
• Adhere to all policies and standard operating procedures for care management.

Requirements

• A Bachelor of Arts or Science degree with two years of relevant experience.
• A license as a Registered Nurse with two years of relevant experience.
• A master’s degree with one year of relevant experience.
• CNAs with two years of experience and a bachelor's degree.
• Experience with direct care for people with intellectual and developmental disabilities.
• Experience with children with intellectual and developmental disabilities.
• Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
