About the Role
Som Medical Practice is seeking a passionate and skilled Nurse Practitioner to join their team in Niagara Falls, Olean, Lockport, and Hamburg. This role offers an opportunity to impact patient lives within a supportive clinical environment.
Responsibilities
- Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations.
- Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines.
- Develop and manage individualized patient care plans and treatment protocols.
- Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies.
- Prescribe medications and provide patient education on medication management.
- Collaborate with physicians, specialists, and care team members to ensure continuity of care.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
Requirements
- Active Nurse Practitioner (NP) license in the state of New York.
- Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program.
- Current board certification (FNP-C, ANP-C, or relevant specialty certification preferred).
- Valid DEA license and prescriptive authority in New York State.
- Experience in a primary care, internal medicine, or outpatient clinical setting preferred.
- Proficiency with electronic health record (EHR) systems.
- Strong interpersonal and communication skills with a patient-centered approach.
