About the Role

Counsel individuals, groups, or families to diagnose and treat mental health disorders, referring clients to specialists, monitoring progress, and creating a supportive treatment environment.

Responsibilities

• Conduct regular appointments with clients (video and/or telephonically).
• Establish positive and trusting relationships with clients.
• Screen and assess patients for common mental health and substance use disorders using evidence-based tools.
• Provide patient education about common mental health and substance use disorders and available evidence-based treatments.
• Implement various treatments and protocols using evidence-based techniques.
• Continually assess client situations and provide proper ongoing treatments.
• Educate and involve family members or loved ones in the treatment process when necessary.
• Partner with plan and community resources to make referrals to outside sources or agencies.
• Coordinate care after an acute event in collaboration with the patient’s provider and population health team.
• Coordinate with the patient’s care plan team to ensure holistic care.
• Maintain the strictest confidentiality of each client situation.
• Maintain all required licenses and appropriate insurance.
• Participate in clinical team meetings as assigned.
• Practice in the office in New York.

Requirements

• Must have an Independent License.
• Must have at least 2 years of therapy experience.
• Bilingual Spanish and ability to pass language testing.
• Experience with population management.
• Must already have a license to practice independently in New York.
• Internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload for remote work.
• Qualified applicants with criminal histories will be considered for employment in a manner consistent with all federal, state and local ordinances.
