About the Role

The Transplant Coordinator is a Registered Professional Nurse responsible for coordinating clinical programs and services for designated patient populations within the Transplantation Institute.

Responsibilities

• Coordinates the multidisciplinary screening and evaluation of patients.
• Ensures all necessary lab tests are performed and results are updated.
• Participates in recommendations regarding placing patients on the transplant waiting list.
• Educates patients on the benefits of living donation.
• Communicates with active transplant candidates regarding waiting list status.
• Ensures all medical clearances are updated regularly.
• Facilitates all organ offers from UNOS, communicating with surgeons, fellows, and patients.
• Answers patient calls after hours, on holidays, and weekends.
• Follows patients during hospitalization and coordinates with the transplant team.
• Coordinates outpatient follow-up visits.
• Educates patients and families regarding transplant medications, lifestyle changes, and outpatient follow-up protocols.
• Assesses continued recovery of patients through compliance with outpatient visits and lab reviews.
• Adjusts immunosuppressive medication as needed.
• Supports the mission, philosophy, goals, and objectives of the Department of Nursing and The Mount Sinai Medical Center.
• Maintains clinical competence.
• Is responsible and accountable for their own nursing practice.
• Advocates for the highest standards of nursing practice.
• Approaches patients, families, and other members of the health care team in a professional, respectful manner.
• Serves as a clinical role model and resource to nursing staff.
• Serves as a professional resource to other members of the health care team.
• Demonstrates professional involvement by attending and participating in workshops, seminars, and courses as presenter or author.
• Keeps abreast of current literature and trends in practice.
• Participates in related professional organizations.
• Considers ethical issues in professional nursing practice.
• Treats co-workers, patients, and families with dignity and respect.
• Maintains attendance and punctuality record.
• Adheres to dress code.
• Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care.
• Demonstrates knowledge of hospital, departmental, and care center standards, policies, procedures, and guidelines.
• Participates in the development and implementation of continuous quality improvement programs.
• Participates in risk management activities.
• Participates in committee activities.
• Is aware of and responsive to budgetary constraints.
• Assists in the recruitment/retention, annual evaluations, and disciplinary process for support staff.

Requirements

• Baccalaureate Degree in Nursing is required.
• NYS Current RN licensure.
• Progressive job related experience specific to area of responsibility.

Education Level

• Baccalaureate Degree in Nursing
• Masters in Nursing and certification in specialty area are preferred

• $107999.99 - $153000 Annually
