About the Role
Oak Street Health, a part of CVS Health, helps older adults stay healthier and live fuller lives. As a Population Health Manager, you will coordinate and implement key Population Health priorities, working on programs and interventions to improve quality and cost of care. This role requires cross-team leadership and collaboration with providers, care teams, and operations members to drive innovation and improvement within a geographic division.
Responsibilities
- Lead strategic planning, execution, and refinement of Population Health programs in the respective region.
- Collaborate with regional leaders and center care teams to support the Oak Street care model, including clinical documentation excellence, workflow optimization, and avoidable admission prevention.
- Develop scalable solutions for accurate clinical documentation and avoidable admission prevention strategies across multiple centers.
- Partner with functional leaders in the central office to assist with strategy and program design for clinical documentation and medical cost/admission efforts.
- Lead analytics, insights, and project management to drive population health execution in the region.
- Manage medical scribe cohort recruitment, retention, performance, and engagement to ensure clinical documentation excellence.
- Achieve key population health metrics, including quality, utilization, and financial measures.
- Perform other duties as assigned.
Requirements
- Minimum of 3 years of applicable experience in healthcare, population health, or working with healthcare providers.
- Strong communication skills.
- Ability to build trust and confidence with a wide range of stakeholders.
- Leadership potential and ability to guide team members in executing population health priorities.
- Demonstrated history of creative problem-solving.
- Strong organization and detail orientation skills.
- US work authorization.
- Qualified applicants with arrest or conviction records will be considered in accordance with all federal, state and local laws.
