About the Role

The Community Health Worker is responsible for identifying high-risk prenatal and postpartum women and assisting them in gaining access to primary and preventive health services, including prenatal care, PCAP, WIC, and other essential and supportive services as needed.

Requirements

  • High School Diploma or equivalent.
  • Valid New York State Driver's license.
  • Personal car to carry out employment duties.
  • Active automobile insurance policy.
  • Basic computer skills.
  • Good writing and verbal skills.

  • Spanish or Creole speaking a plus.
