About the Role

A Self Directed Services Coordinator drives operational, financial, and regulatory integrity for a 80-person caseload in OPWDD Self-Directed Services—empowering individuals to maximize their independence while maintaining compliance with state and federal healthcare standards.

Responsibilities

  • Overseeing a caseload of up to 80 individuals.
  • Processing all reimbursements for individuals, vendors, and staff, ensuring compliance with Medicaid and OPWDD requirements.
  • Reviewing all staff notes on a monthly basis.
  • Monitoring expenses and reviewing monthly expenditure reports, sharing reports with each individual’s team.
  • Acting as the main point of contact for individuals’ self-directed services.
  • Attending biannual Life Plan meetings and reviewing Life Plans for compliance with Medicaid and OPWDD requirements for service delivery and billing.
  • Reviewing Staff Action Plans to ensure alignment with the Life Plan.
  • Communicating with various Wildwood departments (HR, Finance, Training) involved in a person’s Self-Direction services.
  • Maintaining annual required OPWDD and Wildwood trainings.
  • Ensuring all documentation is scanned and saved for 10 years, regularly auditing individual files, and maintaining organization of electronic files.
  • Staying updated on OPWDD and Medicaid regulations for Self-Direction services and communicating them to participants.
  • Compiling information for individuals receiving a Housing Subsidy, which may include Quality Assurance walkthroughs, home visits, assisting in gathering documents, and completing monthly progress notes for billing.

Requirements

  • Bachelor's Degree in a Human Services-related field preferred.
  • Associates’ Degree and one-year of experience working with people with Intellectual and Developmental Disabilities may be substituted for a Bachelor's Degree.
  • QIDP Preferred.
  • A valid NYS driver's license.
  • Daily access to a vehicle.
  • Driving record acceptable to the agency insurance carrier's standards.
