Responsibilities
- Provide assistance with activities of daily living, including personal care, meal preparation, medication administration, and health maintenance.
- Support individuals in developing independent living skills, such as money management, community integration, and social skills.
- Encourage and provide advocacy for individuals.
- Support the Direct Support Professional Supervisor with mentoring and training Direct Support Professionals.
- Provide Residential Habilitation and Day Habilitation services in homes and the community.
- Ensure the health and safety of individuals, maintaining staffing ratios and providing flexible coverage across shifts, programs, and sites.
- Assist individuals with achieving desired personal outcomes.
- Support individuals in achieving optimal health, including medical appointments, medication administration, and meal planning.
- Ensure routine household maintenance, including meal preparation, cleaning, and laundry.
- Support individuals with all personal care needs, including hands-on assistance.
- Help individuals develop skills to increase independence.
- Serve as a role model, demonstrating appropriate social interactions and implementing crisis intervention techniques.
- Complete documentation and safety checks, utilizing computer applications and paper documentation.
- Assist with scheduling of direct care staff.
- Provide transportation for individuals to various community sites using an agency vehicle.
- Provide mentoring, training, oversight, and feedback to Direct Support Professionals.
- Perform other duties as needed or assigned.
Requirements
- Demonstrate all skills and attributes outlined in the NYS DSP Core Competencies.
- Ability to work in a diverse and inclusive environment with individuals of various abilities.
- Complete all mandatory trainings within the first 90 days and maintain ongoing recertifications.
- Professionally and effectively communicate both verbally and in writing.
- Understand regulations and document services in real-time using electronic technology.
- Ability to multi-task and prioritize job responsibilities.
- Respond timely and effectively to emergency and crisis situations.
- Be proactive in identifying, reporting, and resolving safety/medical concerns.
- Ability to accurately count money and complete basic math and banking transactions.
- Complete training to become an approved medication administration professional (AMAP) certified and accurately administer medications.
- Participate in team meetings and service plan meetings as necessary.
- Ensure service plans are written in a timely and accurate manner following life plan meetings.
- Provide person-centered, culturally competent services.
- Provide continuous care and maintain approved staffing ratios at all times, including weekends and holidays.
- Provide on-call coverage for the primary site and a cluster of sites as assigned.
- Provide leadership and mentorship to a team of DSPs working in a residential setting.
- Ability to access reliable transportation.
- Maintain a valid, unrestricted, insurable driver's license.
- Must be capable of performing all SCIP Interventions.
- Able to provide hands-on care to individuals with various physical disabilities.
- Ability to perform various household cleaning tasks.
- Occasional lifting of 25+ pounds, occasional pushing/pulling of 25+ pounds, and occasional reaching and bending.
- Ability to ascend and descend stairs.
- Ability to participate in light to moderate physical activities.
- Ability to utilize various electronic devices.
- Provide transportation utilizing vehicles ranging in size from sedans to large passenger vans.
- Exposure to pets may be required at certain sites.
