About the Role

The Patient Navigator performs a variety of tasks to assist patients in maintaining relationships with providers and other services.

Responsibilities

  • Promotes a safe, cooperative, and professional healthcare environment.
  • Adheres to all hospital policies, procedures, rules, and regulations.
  • Prioritizes customer service, treating all individuals professionally with courtesy and tact.
  • Effectively communicates SBH services and ensures concerns reach the appropriate personnel.
  • Demonstrates standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
  • Conducts outreach to encourage patient engagement and close care gaps in HIV primary care, HIV prevention, and Infectious Disease programs.
  • Consults and collaborates with providers, case managers, social workers, and others to ensure patient access to appointments, medications, equipment, and services.
  • Assists patients in transitioning smoothly between care locations, especially after ED visits or inpatient admissions.
  • Ensures patients have required elements for effective provider visits prior to appointments.
  • Provides appointment reminders for Pathways services and follows up on missed appointments.
  • Links newly diagnosed HIV positive individuals to HIV primary care via referral or escort to the Pathways clinic.
  • Connects patients with appropriate SBH resources and community resources.
  • Accurately documents all service-related activities in the Center’s electronic health record.
  • Participates in program and/or departmental quality improvement activities.
  • Performs assigned tasks by the Program Director related to departmental activities.
  • Attends scheduled department and division meetings and participates appropriately.

Requirements

  • Three years of experience in relevant care coordination services, which can include Medical Assistant experience.
  • Excellent communication skills, verbal and written.
  • Ability to interact with a variety of medical providers and allied health staff.
  • Working knowledge of physical health.
  • Proficiency in electronic data collection systems.
