About the Role

The HLPS Case Manager develops and executes housing navigation and placement services, including follow-up, for clients referred by NYC Health + Hospitals. This role involves creating housing plans, assisting clients in finding and moving into permanent housing, and providing ongoing support post-placement. The Case Manager will document all interactions, coordinate services with relevant contacts, and play a key role in identifying client needs, monitoring progress, and resolving issues.

Responsibilities

  • Conduct intake process with new clients within 48 hours of assignment, including explanation of client rights, responsibilities, and grievance procedures.
  • Develop initial housing plans and conduct reviews as needed for an assigned caseload.
  • Assess eligible housing vouchers, identify housing preferences, search for opportunities, coordinate housing tours/interviews, collect documents, and complete applications.
  • Schedule regular follow-ups after initial placement to identify support needs, troubleshoot incidents, confirm payment, and assist with voucher reapplication and lease renewals.
  • Provide accurate, complete, and timely data entry and documentation of client housing status, interactions, communications, social services, and progression toward securing permanent affordable housing.
  • Attend staff meetings, case conferences, group supervision, and professional development activities.
  • Work with other consortium partners as necessary.

  • Employ best practices for effective feedback and communication at all levels of staff engagement.
  • Perform all job-related tasks as required.

Requirements

  • High School Diploma or Equivalent.
  • Bachelor’s degree in human services field preferred.
  • Minimum of two years of experience in case management, tenant organization, counseling, community outreach and/or direct advocacy for clients in social service settings.
  • Experience working with homeless individuals and people living with health and/or mental health issues and substance use is a plus.
  • Ability to engage with residents.
  • Knowledge of applicable state and city housing and disability laws and codes preferred.
  • Knowledge and experience working with NYC voucher programs, housing subsidies and supportive housing programs preferred.
  • Computer literacy with advanced knowledge of Word, Excel, and Outlook.
  • Familiarity with HOME, CARES, CAPS, Housing Connect, etc. databases.
  • Willingness to travel throughout the five boroughs and make multiple visits to the community as needed.
  • Sporadic evening or weekend hours may be required.
  • Demonstrated commitment to quality and data-driven reporting.
  • Track record of effective collaboration within a direct service organization balancing a large caseload with competing priorities.
  • Strong written and verbal communication skills.
  • Action-oriented, entrepreneurial, flexible, and innovative approach to interacting with and supporting clients.
  • Bilingual (or multi-lingual) is preferred.
