Responsibilities
- Participates in screening of clients' medical needs, gathering admission information, and observing clients' physical, mental, and emotional status.
- Works cooperatively with managerial and medical staff to meet client treatment needs.
- Provides group counseling sessions to clients regarding health-related issues.
- Observes that all medications are properly taken by clients and documents them in MAR and 10e11.
- Takes client vitals as ordered by a doctor.
- Gathers and submits urine drug screens to labs.
- Documents lab results in client 10e11 charts in a timely fashion.
- Monitors clients for medical needs and reports findings to the medical provider.
- Ensures confidentiality and safety/security of all residential clients.
- Logs and verbally receives/delivers accurate information at shift changes.
- Prepares necessary reports and records as required by program procedure.
- Participates in weekly case conferences as needed.
- Assumes responsibility for reporting incidents involving client medical occurrences.
- Makes calls to local ambulance service, fire department, police, or CDPC, as needed.
- Attends required staff meetings and training.
- Performs other duties as assigned and works in other residential buildings as needed.
Requirements
- Candidate must have a valid NYS LPN or R.N. license.
- Candidate must have a valid NYS Driver's License.
- Must be CPR and First Aid certified.

- Two (2) years nursing experience preferred, preferably in a NYS OASAS residential facility.
