About the Role

The Client Relationship Specialist is an LPN responsible for developing and maintaining client relations and intake of direct patient referrals made to HCR.

Responsibilities

  • Cultivate relationships with physicians and hospitals to facilitate the best Home Care solutions for patients.
  • Monitor HCR RightFax folders, label faxes, and attach electronic documents to patient records in Home Care Home Base (HCHB).
  • Monitor Intake Team email.
  • Produce timely and accurate data entry.
  • Enter orthopedic prescreens into HCHB according to agency protocol.
  • Provide phone support to the department and triage calls to Intake staff.
  • Provide excellent customer service to internal and external customers.
  • Participate in department and agency meetings.
  • Perform other duties as assigned.

Requirements

  • Minimum of 4 years of experience in a clinical setting.
  • Exceptional customer service and communication skills.
  • Highly proficient and efficient working with a computer.
  • Detail-oriented.
  • Able to work in a fast-paced environment.
