Responsibilities
- Coordinate care for resident services, providing education, support, and accountability.
- Prepare assessments and develop/implement individual service plans.
- Assist residents in achieving goals and facilitate groups and activities.
- Teach and assist residents in developing ADA skills, communication, and self-advocacy skills.
- Facilitate resident involvement and community activities.
- Make referrals to community-based services and assist in accessing/maintaining entitlements/benefits.
- Advocate for needed services and assist residents in meeting tenancy obligations.
- Meet regularly with the Senior Housing Supervisor for file documentation review.
- Maintain accurate case file records and reports based on funding source requirements.
- Facilitate the integration of residents into the community.
- Ensure licensed apartments are maintained at standards for apartment recertification.
- Ensure resident fees related to rent and phone bills are paid in a timely manner.
- Assist residents in complying with treatment/service providers.
- Serve as an advocate/mediator for residents experiencing difficulties with landlord/tenant disputes or provider, employment, funding related issues.
- Act as a resource and assist residents transitioning from the program to a more/less restrictive level of care.
- Conduct routine housing inspections to assess resident needs and identify those requiring assistance.
- Attend staff meetings, treatment team/provider meetings, and staff development/training seminars.
- Conduct housing-related screening, client intake, and assessments.
- Ensure all program criteria and eligibility are met.
- Assess, advocate, and provide linkages to community-based services.
- Process Catholic Charities residential housing admissions.
- Develop relationships with area landlords to facilitate recipient placement.
- Assist recipients with the housing search process, including identifying housing options, accompanying recipients to view units, advocating with landlords, and assisting with paperwork.
- Complete other duties as assigned.
Requirements
- Associates Degree in human services field or related field preferred.
- Two years’ experience in Human Services or related field preferred.
- A combination of education and experience may be substituted at the discretion of the Executive Director.
- Ability to use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle.
- Ability to stand, sit, walk, bend, stoop, kneel, and reach.
- Ability to climb up or down stairs.
- Ability to reach above or below shoulders.
- Ability to occasionally lift or move objects weighing up to 10 lbs.
- Ability to sit at a desk or in a vehicle for long periods of time.
- Ability to read, write, and interpret written documents.
- Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing, and documentation standards.
