About the Role

The Housing Case Manager (HCM) plays a core role in coordinating care for resident services, providing education, support, and accountability to ensure residents receive necessary services and the program remains compliant with all regulations.

Responsibilities

  • Coordinate care for resident services.
  • Provide education, support, and accountability to residents.
  • Prepare assessments and develop/implement individual service plans.
  • Facilitate groups and activities.
  • Teach and assist residents in developing essential life skills.
  • Make referrals to community-based services.
  • Assist residents in accessing and maintaining entitlements/benefits.
  • Advocate for needed services.
  • Assist residents in meeting the obligations of tenancy.
  • Meet regularly with the Senior Housing Supervisor for file review.
  • Maintain accurate case file records and reports.
  • Facilitate resident integration into the community.
  • Conduct routine housing inspections and identify residents needing assistance.
  • Attend staff meetings, treatment team meetings, and training seminars.
  • Complete other duties as assigned.

Requirements

  • Associate’s degree in human services field or related field preferred.
  • Two years’ experience in Human Services or related field preferred.
  • Relevant combination of education and experience will be considered.
  • Ability to use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle.
  • Ability to stand, sit, walk, bend, stoop, kneel, and reach.
  • Ability to reach above or below shoulders.
  • Ability to occasionally lift or move objects weighing up to 10 lbs.
  • Ability to sit at a desk or in a vehicle for long periods.
  • Ability to read, write, and interpret written documents.
  • Adherence to all applicable federal and state laws and regulations.
  • Successful completion of a background check is required.
