About the Role
The Exercise Physiologist supports the Sutter Sync HF multidisciplinary model by delivering remote cardiac rehabilitation and individualized exercise planning for heart failure patients. This role owns the exercise component of the joint frailty intervention, uses activity and functional-status signals to identify decline, and partners closely with Nutrition, Nursing, Pharmacy, and MD teams to support safe reconditioning after decompensation.
Responsibilities
- Deliver remote cardiac rehabilitation and individualized exercise planning for heart failure patients.
- Own the exercise component of the joint frailty intervention.
- Use activity and functional-status signals to identify decline.
- Partner closely with Nutrition, Nursing, Pharmacy, and MD teams to support safe reconditioning after decompensation.
Requirements
- Bachelor's degree in exercise physiology or related field.
- BLS-Basic Life Support Healthcare Provider certification.
- 5 years of recent relevant experience.
- Knowledge of anatomy, physiology and pathophysiology, chemistry and physics.
- In-depth knowledge of exercise testing, exercise equipment and quality control.
- Communication and teaching skills.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Ability to perform multiple tasks and prioritize independently with follow through in a timely manner.
- Ability to design, implement, and supervise individualized exercise prescriptions for people with chronic disease and disabling conditions.
- Ability to respond appropriately to emergency situations.
- Collaborative working style to facilitate open sharing of information.
