About the Role

Seeking a compassionate, skilled, and patient-focused Licensed Practical Nurse (LPN) to join our Dialysis Department. Under the direction of a qualified Dialysis Registered Nurse, the Dialysis LPN provides direct patient care and supports the delivery of high-quality hemodialysis services, making a meaningful impact in patients' lives within a collaborative environment.

Responsibilities

  • Provide direct patient care under the supervision of a qualified Dialysis RN.
  • Monitor patients during dialysis treatments and report clinical observations to the RN.
  • Assist in implementing individualized patient care plans.
  • Perform therapeutic and preventive nursing interventions within the LPN scope of practice.
  • Conduct patient and family health teaching and counseling.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Adhere to established policies, procedures, and regulatory standards.
  • Promote patient safety, dignity, and well-being at all times.
  • Support a positive team environment through effective communication and collaboration.
  • Maintain confidentiality of patient and organizational information.

Requirements

  • Graduate of an accredited School of Practical Nursing.
  • Current New York State Licensed Practical Nurse (LPN) license.
  • Basic Life Support (BLS) certification through the RQI system during clinical orientation.
  • IV Therapy Certification after one year of nursing practice.
  • Minimum one (1) year of acute care nursing experience.

  • Demonstrated professionalism and excellent customer service skills.
  • Ability to maintain confidentiality and exercise discretion.
  • Computer proficiency and ability to work within electronic medical record systems.
  • Experience with Microsoft Office applications preferred.
  • Commitment to teamwork and positive role modeling.
  • Knowledge of renal disease processes and hemodialysis procedures.
  • Ability to effectively assess and respond to the needs of adolescent, adult, and geriatric patient populations.

Experience Level

  • Minimum one (1) year of acute care nursing experience.
  • One (1) year of experience in Dialysis, Intensive Care Unit (ICU), Coronary Care Unit (CCU), Emergency Room (ER), or Operating Room (OR) preferred.

Education Level

  • Graduate of an accredited School of Practical Nursing.
