About the Role

The Medical Coordinator / Shift Supervisor is responsible for coordinating medical and health appointments, ensuring individuals receive necessary treatments and care according to regulations and agency mission. This role oversees health follow-up, treatments, referrals, and prescribed protocols, maintaining accurate medical documentation and acting as a liaison between various parties. The position supports individuals with intellectual disabilities and mental health needs in reaching their goals and promotes a safe, therapeutic environment that fosters growth and rehabilitation.

Responsibilities

  • Supervise and manage the day-to-day operations of a Residential Program.
  • Ensure staff are assigned to tasks, individual supervision levels, and activities according to operational procedures.
  • Effectively communicate and support the organization’s mission, vision, and values.
  • Monitor and ensure daily documentation for individuals’ programming is complete and accurate.
  • Ensure goals are met in areas including client and family satisfaction, safety, quality, and team member performance.
  • Conduct compliance reviews of medical and program records.
  • Develop systems to ensure the health and safety of individuals and identify risks.
  • Serve as the primary contact for doctor’s offices and medical staff regarding programming and health information.
  • Oversee the physical plant of the facility, including daily walk-throughs and maintenance.
  • Ensure program vehicles are maintained and staff follow driving procedures.
  • Conduct morning, meal, and medication observations to ensure adequate supervision and adherence to safeguards.
  • Implement residential schedules and procedures to ensure safety and organization.
  • Observe programming and mealtimes to ensure individuals' safeguards are followed.
  • Direct staff in planning and coordinating services to teach life skills and increase independence.
  • Attend and participate in required meetings, staff development, and training programs.
  • Collaborate with the IDT team to ensure continuity of care and follow up on recommendations.
  • Work with clinical teams to ensure residential services incorporate adequate health and medical procedures.
  • Meet with the Registered Nurse weekly to review follow-ups, medication stock, and supplies.
  • Accompany individuals on medical appointments and advocate for quality healthcare.
  • Ensure individuals are appropriately dressed and groomed and safeguards are implemented.
  • Ensure all individual medications are administered as prescribed and records are accurately documented.
  • Ensure lunches and snacks are appropriate to individual diets and meal plans.
  • Uphold Direct Care Responsibilities when working with individuals or providing staff coverage.
  • Ensure individual body weights are taken weekly and data is maintained.
  • Ensure all required medical appointments are scheduled and occur as ordered.
  • Maintain all Consultation Visit Summaries.
  • Maintain organized and well-kept medical records and medication administration records.
  • Accompany individuals on medical appointments as necessary and address team recommendations.
  • Ensure all medications are ordered timely and as ordered by the physician.
  • Communicate with staff and management to ensure physician orders are carried out.
  • Attend medical appointments with individuals to relay information and advocate for them.
  • Maintain effective communication with physicians, doctors’ offices, and pharmacists.
  • Ensure Medication Administration Record follows OPWDD and agency compliance.
  • Monitor Medication Administration Stations to ensure proper medication administration.
  • Cover duties and leadership roles in the event of a vacant position or leave time.
  • Work on all other projects as assigned by supervisor(s).

Requirements

  • Ability to interact with individuals and staff professionally and ethically.
  • Ability to understand and implement service plans.
  • Ability to complete all required documentation clearly, legibly, and in a timely manner.
  • Passion for and desire to work with people with intellectual disabilities and other physical or mental disabilities.
  • Knowledge of basic human physical and psychological needs.
  • Knowledge of simple health and safety precautions.
  • Knowledge of simple nursing care, first-aid, behavior management, personal and environmental hygiene.
  • Ability to write progress and behavior reports using a professional, objective style and voice.
  • Ability to act as a liaison between program planning teams.
  • Ability to use computers and other electronic equipment.
  • Ability to interact with individuals, community members, and other treatment team members.
  • Ability to read and comprehend technical training manuals.
  • Ability to follow written and oral instructions.
  • Ability to learn basic operation and maintenance of adaptive equipment.
  • Ability to be supportive and patient.
  • Ability to be a team player.
  • Ability to observe, record, and report participant’s behavior, attitude, and physical condition.
  • Ability to promote self-advocacy for individuals during medical assessments.
  • Ability to work extended day-to-day hours.
  • Ability to prioritize conflicting deadlines.
  • Ability to obtain SCIP, First Aid/CPR certification and Medication Administration Training.
  • Attention to detail and structured way of approaching tasks.
  • Must have a valid New York State Driver’s License.
