About the Role

Peer Support services are peer-delivered services with a rehabilitation and recovery focus, designed to promote coping skills and enhance recovery-oriented principles. These services use trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder, supporting individuals and families in initiating and maintaining recovery.

Responsibilities

• Provide advocacy for individuals seeking and obtaining benefits, entitlements, food, and shelter.
• Assist recipients in participating in shared decision-making.
• Utilize Peer Bridgers to assist with linkages and systems navigation within behavioral health and allied human services.
• Offer benefits advisement and planning.
• Assist in the development of psychiatric advance directives (PAD).
• Provide companionship and model a recovery lifestyle.
• Raise awareness of existing services and help individuals remove barriers to access.
• Conduct interim visits with individuals and/or their families after discharge to facilitate community tenure.
• Assist in selecting and utilizing self-directed recovery tools and activities that bring passion, purpose, and meaning.
• Connect individuals to natural supports, warm lines, and self-help groups.
• Provide recovery education and counseling for individuals and their family members.
• Offer one-to-one peer support and person-centered goal planning.
• Assist with skills development for a more independent life.
• Provide transitional support, bridging individuals from institutions or jail to home and community.
• Make arrangements for appropriate services prior to discharge.
• Provide pre-crisis and crisis support services, including companionship and developing crisis diversion or relapse prevention plans.

Requirements

• High school diploma or equivalent.
• "Lived" / relevant experience.
• Clean driving record for 3 years.
• Must maintain certification requirements.
