As a Clinical Assessment Manager, you will play a critical role in the participant journey by conducting comprehensive assessments, identifying care needs, and helping connect older adults with the services and support they need to live healthier, more independent lives. Responsibilities
- Conduct comprehensive in-home clinical assessments utilizing the UAS-NY.
- Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
- Perform reassessments and identify changes in condition, functional status, diagnoses, and medication regimens.
- Develop proposed service plans and recommendations to support participant health and independence.
- Provide hands-on services in participants’ homes as needed, including, but not limited to, administering medications via appropriate routes, performing wound care, and carrying out other clinical procedures within the scope of practice.
- Collaborate with Enrollment, Intake, Care Delivery, and IDT teams.
- Document clinical findings accurately and timely within electronic medical record systems.
- Participate in quality improvement initiatives and serve as a clinical resource throughout the assessment process.
Requirements
- Current, active, unrestricted New York State Registered Nurse license.
- Minimum of 1-2 years of clinical nursing experience.
- Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related healthcare settings.
- Strong clinical judgment, communication, and documentation skills.
- Ability to travel throughout The Bronx & Westchester.
