About the Role
Clarity Wellness Community is seeking a full-time Home-Based Crisis Intervention (HBCI) Provider to support children and families. The role focuses on providing intensive, short-term support to prevent unnecessary hospitalizations for youth aged 5-21, utilizing evidence-based practices for safety planning and resource connection.
Responsibilities
- Provide flexible, family-centered services by scheduling sessions at convenient times.
- Conduct timely intakes, assessments, and individualized treatment plans focused on recovery and strengths.
- Offer crisis intervention, de-escalation, safety planning, and motivational support to youth and families.
- Collaborate with families and community providers to communicate care, set goals, and connect clients to appropriate resources.
Requirements
- Master's degree in Psychology, Social Work, Mental Health, or a related field.
- Must be licensed by the New York State Education Department and operate within the practitioner's scope of practice.
- Licensed mental health providers include: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, and Licensed Creative Arts Therapists.
- Unlicensed MA staff with at least one year of relevant experience in community-based mental health or case management may be considered.
