About the Role
The Adult Health Home Care Coordinator works with individuals to develop and maintain a comprehensive plan for accessing and using services and supports necessary for their health.
Responsibilities
- Conduct outreach and assessment for Health Home services for individuals who are Medicaid Eligible and/or have active Medicaid.
- Conduct eligibility screenings and Health Home enrollment.
- Collaboratively work with enrollee to create a comprehensive person-centered plan of care to meet their medical, behavioral health and social service needs.
- Responsible for health promotion, education, and overall coordination of all aspects of an individual's care.
- Offer family support, and referral/linkages to community, social, and other support services as needed.
- Leads multidisciplinary teams of medical, mental health, chemical dependency treatment providers, social workers, nurses and other care providers to assuring enrollees receive services.
- Assist individual in keeping appointments and monitor service delivery.
- Provide Care Management services at various locations including in community locations and enrollees’ homes.
Requirements
- Bachelor's degree required, preferably in Human Service, Forensic, Psychology, Public Health, or related concentration.
- CASAC and two years’ experience working with individuals with mental health, substance use or co-occurring disorders, required.
- Must have a valid driver’s license and own transportation.
- Employees who drive personal vehicles for work must maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and provide HR with proof of current minimum insurance coverage on their first day of employment.
- Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date.
