About the Role

Responsible for patient care activities and goal achievement through assessment, diagnosis, planning, intervention, and evaluation. This role involves health teaching, discharge planning, and collaborating with a care team to ensure optimal patient outcomes.

Responsibilities

• Conducts Nursing assessment based on interview, examination, observation and review of patient records.
• Ensures patient progress is reflected in progress notes and revised care plans.
• Performs discharge planning consistent with patient status and inter-departmental communication.
• Communicates information to patients based on their level of awareness and readiness to learn.
• Supports orientation of new staff members by providing feedback and acting as a preceptor, role model, and resource.
• Functions as a professional role model and mentor for students and volunteers.
• Organizes patient care assignments to meet priority needs based on patient status and personnel skill level.
• Communicates patient and unit conditions requiring additional intervention to leadership.
• Participates in the development of other staff members.
• Communicates with peers to appraise practice related to self and other nursing personnel.
• Demonstrates collaborative effort in meeting patient and unit needs through communication and nursing actions.
• Provides direction, assistance, and support.
• Assumes accountability and responsibility for the completion and quality of assigned tasks.
• Identifies and communicates the need for further staff education to immediate supervisor.
• Communicates with nursing leadership, reflecting awareness of authorities and responsibilities.
• Seeks and utilizes nursing leaders as role models and resource persons.
• Collaborates with other disciplines to meet patient and unit needs.
• Communicates and interacts with families and other groups, reflecting the philosophy, goals, and objectives of the Department of Nursing and the Hospital Center.

Requirements

• Graduation from an accredited Nursing program.
• Bachelor’s Degree in Nursing (BSN).
• Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year.
• Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
• Documented previous experience or educational preparation to support credentials.
• Special skills/experience may be required in specialized areas.
• RN (1 Year) license issued by DOH / Office of Professions.
• ACLS certification required for specific departments (ED; PACU/ASU; ICUs; Telemetry Units; SDU; IR, Interventional Cardiology (CCL/EP/Echo); L&D; Endoscopy, and APN Adult Oncology) from the American Heart Association.
• BCLS certification from the American Heart Association.
