About the Role

Provides personal care and assistance to members with activities of daily living as directed by the RN Health Care Coordinator. Will also assist and support with daily recreation therapy programs.

Responsibilities

• Performs personal care and assistance with activities of daily living as directed by the RN Health Care Coordinator or other nursing staff.
• Documents care and assistance in the medical record.
• Reports changes in member’s condition to the Registered Nurse, staff Licensed Practical Nurse, Program Director or Social Worker as appropriate.
• Maintains members confidentiality.
• Assists with meal service.
• Assists with recreation therapy programs throughout the day per direction from Rec Therapist or ADHC Director.
• Ensures personal care items are stocked in the clinic area.
• Performs other tasks necessary for the effective operation of the program, e.g., answering the telephones, running errands and taking and delivering messages.
• All other job duties as assigned, including required floating assignment to nursing home when needed.
• All other job duties as assigned.

Requirements

• Must be able to read, write and speak the English language in an understandable manner.
• Must be able to work independently, make good observations and have good reporting skills.

Experience Level

• Current New York State Certified Nursing Assistant Certification
• High school diploma or equivalent required.
