About the Role
The HHA is responsible for providing assigned residents with routine daily nursing care and services in accordance with their individual service plan and as directed by supervisors. The role requires administrative authority, responsibility, and accountability for carrying out assigned duties.
Responsibilities
- Documents all cares provided in EMR.
- Reports all changes in the residents condition to the LPN.
- Agrees not to disclose assigned user ID code and password for accessing resident/facility information and promptly reports suspected or known violations.
- Agrees not to disclose residents protected health information and promptly reports suspected or known violations.
- Reports any known or suspected unauthorized attempt to access facilitys information system.
- Performs all assigned tasks in accordance with established policies and procedures, and as instructed by supervisors.
- Cooperates with inter-departmental personnel and other facility personnel to ensure nursing services adequately meet resident needs.
- Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
- Meets with nursing personnel regularly to identify and correct problem areas or improve services.
- Reports all complaints and grievances made by the resident.
- Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to supervisor.
- Reports known or suspected incidents of fraud to the Administrator/Corporate Compliance Officer.
- Ensures departmental computer workstations left unattended are properly logged off or password protected.
- Participates in and receives the nursing report for duty.
- Assists resident with all grooming tasks each shift, including oral/dental care, bathing, dressing, hair care, shaving, and nail care.
- Assists resident with bowel and bladder functions and any incontinence care, ensuring residents are dry.
- Assists in transporting residents to/from appointments, activities, and social programs.
- Assists with lifting, turning, moving, positioning, and transporting residents using proper body mechanics.
- Assists residents to walk with or without self-help devices as instructed.
- Weighs and measures residents as instructed.
- Answers residents calls promptly, ensuring residents who are unable to call for help are checked frequently.
- Assists with the care of a dying resident and provides post-mortem care as instructed.
- Makes and changes beds.
- Prepares residents for meals (e.g., take to bathroom, wash hands, comb hair, raise beds, position tables, place clothing protectors or napkins, take to/from dining room).
- Serves food trays and assists with feeding as indicated.
- Assists residents with identifying food arrangements.
- Assists with between meal and bedtime snacks as requested.
- Performs after meal care (e.g., remove trays, clean residents hands, face, clothing, take to bathroom, brush teeth, clean dentures).
- Checks rooms for food articles.
- Attends and participates in scheduled training and educational classes to maintain current certification.
- Attends and participates in scheduled orientation programs and activities.
- Attends and participates in facility in-service training programs.
- Washes hands before and after performing any service for the resident.
- Keeps the nurses call system within easy reach of the resident.
- Immediately notifies the LPN of any resident leaving/missing from the facility.
- Follows established safety precautions in the performance of all duties.
- Reports any communicable or infectious disease to the Director of Nursing Services and/or Infection Control Coordinator.
- Follows established isolation precautions and procedures.
- Wears and/or uses safety equipment and supplies when lifting or moving residents.
- Reports missing/illegible labels and SDSs to supervisor.
- Uses only trained equipment in a safe manner.
- Reports defective equipment to the LPN.
- Informs the LPN of equipment and supply needs.
- Maintains the confidentiality of all residents care information, including protected health information.
- Reports known or suspected incidents of unauthorized disclosure of resident information.
- Treats all residents fairly, with kindness, dignity, and respect, knocking before entering rooms, and ensuring privacy.
- Reports all allegations of resident abuse and/or misappropriation of resident property.
- Honors the residents refusal of treatment request and reports such requests to supervisor.
- Works throughout the nursing system area, intermittently moving due to frequent interruptions.
- Is involved with residents, personnel, visitors, and government agencies under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Works beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
- Is subject to call back during emergency conditions.
- Completes all required training and in-services.
- Performs all other duties as assigned.
Requirements
- High School Diploma or HSE.
- An active and valid HHA certification required.
- Current Nursing Assistant certification in New York State desired.
- Proficiency with computers and standard office software.
- Must be able to read, write, speak, and understand the English language.
- Demonstrated knowledge and skills necessary to provide care primarily to the geriatric population.
