About the Role
We are seeking a skilled and compassionate Pharmacist to join our team. As a Pharmacist you will play a crucial role in providing medication counseling in support of professional medical staff.
Responsibilities
- Completes medication reconciliation and counseling for all initial patients
- Provides patients with symptom and medication counseling on health-related problems as directed by the attending physician
- Takes and records or reviews and interprets the patient's medical and social history
- Screens patients to assess need for immediate care or referral to other health personnel
- Conducts follow-up phone calls with high-risk patients
- Documents in patient charts and maintains detailed records of patient treatment
- Performs related duties as assigned by the medical staff
- Deliver exemplary customer service in order to provide a positive patient experience across the organization
- This position may require meeting with patients in community settings based on clinical needs
Requirements
- Good knowledge of clinical medicine, medical science
- Ability to make preventive and therapeutic medication recommendations based on accurate medical diagnosis
- Ability to relate well to supervising physicians
- Ability to communicate well with and relate well to colleagues and patients
- Physically capable of performing the essential functions of the position with or without reasonable accommodation
- 1-2 years of experience in a clinical pharmacy, preferred
- Basic Life Support (BLS) Certification, required
- Possession of a valid, active license and current registration to practice as a Pharmacist in New York State as issued by the New York State Education Department
Skills
- Proficiency in Electronic Medical Records (EMR), computer skills
- Strong organizational and time management skills with the ability to manage multiple schedules and prioritize tasks
- Excellent interpersonal skills to interact with patients, surgeons, and other healthcare team members
- Strong understanding with insurance verification and authorization process
