About the Role
The Resource Coordinator (Care Manager) is responsible for participating in outreach and enrollment, and providing assistance and guidance to individuals, family members, and caregivers on available services. This role serves on an inter-disciplinary team to manage the medical, social, and behavioral health needs of enrolled members, facilitating access to services necessary for health, hospitalization prevention, and stability.
Responsibilities
- Conduct recruitment efforts and monitor waiting lists to ensure accurate placement.
- Complete intake, enrollment, and ongoing assessments.
- Facilitate interdisciplinary collaboration among providers, enrollees, families, caregivers, and support systems.
- Ensure families are aware of all support services and entitlements.
- Provide recommendations and referrals to relevant community resources.
- Advocate for individuals/families by serving as a liaison between them and other programs.
- Conduct office, home, and community visits to assess family needs and service delivery.
- Perform outreach to program candidates within the community.
- Assist in coordinating services for special needs children with disabilities.
- Monitor and maintain compliance with all Federal, State, and Local regulations.
- Monitor compliance using data management systems.
Requirements
- Ability to plan and carry out assignments independently.
- Ability to prioritize, adhere to timelines, and multi-task.
- Home visits are required.
