About the Role
The Care Coordination program aims to reduce HIV healthcare disparities by providing client-centered, holistic, and comprehensive care management. It utilizes a patient navigation approach to help individuals with HIV access necessary healthcare and social services, improving health outcomes and addressing barriers to care.
Responsibilities
- Provide client-centered, culturally and linguistically competent services.
- Support adherence to scheduled appointments and care regimens.
- Refer clients to appropriate care and provide linkage support.
- Provide accompaniment to healthcare and social service appointments.
- Conduct community-based home visits in various settings.
- Collaborate with key stakeholders, including leadership, clinic staff, and program staff, to ensure effective communication and service delivery.
- Deliver health education curriculum to enhance self-efficacy and health management skills.
- Provide tailored medication adherence support.
- Participate in Housing Works advocacy events.
Requirements
- High school diploma and 2 years of relevant experience in healthcare or social services.
- Ability to interact with interdisciplinary teams including medicine, social work, psychiatry, nursing, administration, and other paraprofessionals.
- Ability to work with and understand issues of people with HIV/AIDS, mental illness, and substance use populations.
- Knowledge of and experience with mediation and conflict resolution.
- Good verbal and written communication skills.
- Embrace a harm reduction approach to working with underserved populations.
- Experience with eSHARE and/or eClinical Works is preferred.
- Bilingual English/Spanish preferred, but not required.
- Excellent written and verbal communication skills.
- Commitment to maintaining the highest levels of confidentiality.
- Demonstrated ability to build trust and respect with patients, staff, colleagues, external contacts, and other Housing Works members.
- Sensitivity to the needs of a culturally and linguistically diverse patient and employee population.
- Demonstrated ability to meet or exceed the Service Excellence Standards of Housing Works.
- Demonstrated ability to utilize electronic medical records (EMRs) and related computer software, and willingness to adapt to evolving technological requirements.
